The objective of Model 3 is to address the lack of integration and continuation of care between different levels of healthcare and ad-hoc social care services, which can particularly affect older people after hospitalization. The aim is to propose a service delivery model that facilitates care being delivered as close to home as possible in the shortest amount of time. Model 3 will implement care plans for the elderly, improve the quality of care, facilitate access to indispensable diagnostics, reduce the number of unnecessary consultations with specialists and hospital admissions, reduce polypharmacotherapy, and improve the use of resources available at the local level.

The World Bank will support the Government on a detailed work plan to monitor the implementation of an integrated care pilot project. Evaluating the pilot project will require novel monitoring and economic evaluation methods that incorporate the realities of evaluating complex interventions.
The World Bank support would include providing the Government with the evidence on determining the effect of the pilot on the household service utilization and evaluating the capacity of providers to deliver quality services, with the end goal of determining economic feasibility of scaling up integrated health care in Poland.
